Found out the bug when you pick up "Dead cat" as Jacob & Easu you get infinite lifes. I have died 100+ times, lost my sanity but did it and memorized all Isaac's and ???'s attacks.
Here's one of my favourite characters of TBOI ...Azazel �� (P.s Can you spot The Lost?)

Item Discussion

When it comes to Isaac items, melee is a niche that sat pretty bare for a while. Naturally, Mom's Knife has been doing some heavy lifting for years, but it wasn't until Afterbirth that we saw another pseudo-melee in the form of Spear of Destiny, and it took AB+ for the gods to shine upon us with a melee character in the form of Mr. Forgotten ^{^{^{^the}}} ^{^{^{^best}}} ^{^{^{^default}}} ^{^{^{^character.}}}

Finally, after a decade of wait, Repentance has us melee fans eating great. For the filthy Bullet mains among us, we've finally got an excuse to run into enemies! We've got Tainted Judas, Samson, Maggy, and Azazel, we've got the Notched Axe rework, and we've got this darned thing.

And so, there only remains one question to be asked: do you like this sword, sword, sword, this Spirit Sword? It is, after all, made of freaking diamonds. Sorry, my brain fell into 2012 for a minute there. What I meant to say was—welcome back to Item Discussion #8, dorks & dweebs. It's Spirit Sword, baby!

Item Effect

• Tears are replaced with a sword which can be swung to deal damage (3 * Damage + 3.5) and knock enemies back

• Holding an attack button charges a spin attack (at 1/4 Isaac's tear rate), which damages all enemies around

• At full red health, each swing fires a projectile across the room (similar to the Master Sword in Zelda) dealing Isaac's Damage + 2 damage to enemies; this has a cooldown of 1/2 Isaac's tear rate
