    Sup guys, its ya boi Turbine here. I'm here to rank all the items, because I'm bored as fuck. I've sunk an ungodly amount of hours (1531, all play time) in to tBOI, and it's my favorite game to just play to in the background while I listen to a podcast of some kind.

    This ranking will be based purely on personal opinion, and can and probably will be edited based on what people think so type in those comments why my opinions are pure shite.

    For each item, I will write my opinion on it, and give it a General Value (GV), which is the item on a standalone basis, from 0-10.

    0: Generally run ending. Don't fucking take this item. It's bad. Avoid like you avoid that coughing dude wearing a mask on the metro.

    1: Generally pretty bad. Unless it gives you great synergies, don't take this item. Can be run-ending.

    2: Pretty bad item but may have its values. Take according to situation.

    3: Usually a bad item, take according to situation.

    4: OK to take but 1. isn't that good or 2. has adverse effects.

    5: Generally take this item, not always. The girl you would make out with while drunk but won't brag to your buddies about.

    6: Good item, unless it ruins a synergy take it.

    7: Good item, take it unless it ruins a very good synergy.

    8: Very good item, take it. Can win you the early floors and in turn the run.

    9: Take take take. Good fucking item. Will win you the early floors unless you do something stupid.

    10: God-tier item. Take it and it will win you the run unless you decide to play like a buffoon in heat.

    I will also give it a Synergy Value (SV) if applicable (for example a general tears up will not have a synergy value since it really just raises your stats), which is based on a variety of things. The synergy value will be based on 1. how hard it is to achieve the synergy and 2. how strong that synergy may be. I will also write on the specific synergy if applicable.

    Items will be written in Item ID order. Here we go! You can open up this website to follow along with the item currently being rated. All item values will be based on general tear firing characters and not for characters with special tears from the start (Azazel, Keeper). All items will be rated on the base game mode.


    1 THE SAD ONION

    ItemID: 1

    "Tears up"

    • +0.7 Tears Up

    TYPE: PASSIVE

    ITEM POOL: ITEM ROOM

    GV: 7

    A very good fire rate upgrade item. Cut them sad onions and cry, baby. Cry faster so the monsters can't get you.

    SV: N/A


    2 THE INNER EYE

    ItemID: 2

    "Triple shot"

    • Tears now shoot three at a time (Triple Shot)

    • Tears Down significantly (tears = tears * 2.1 + 3).

    TYPE: PASSIVE, TEAR MODIFIER

    ITEM POOL: ITEM ROOM

    GV: 3

    Generally, this makes your damage go down. For people who can't shoot straight (like me) though, this might be nice. the missed knockback from a lacked constant stream of firing can also trouble you with rushing enemies.

    SV: 5

    While it can be devastating with good items, the items you are looking for for this synergy generally are items with a GV of 10 anyway. Moms knife, Ipecac, to name a few. Not even that good with brimstone as it will generally add on the tear delay with each other to make you fire slower than granny barehand rockclimbing.


    3 SPOON BENDER

    ItemID: 3

    "Homing shots"

    • Gives Isaac's tears a homing effect.

    TYPE: PASSIVE

    ITEM POOL: ITEM ROOM

    GV: 6

    This one's a bit tough, and some of you might not agree with this general score. Bear in mind though that this is a standalone score. Though homing shots can be nice, it can also curve away from the the intended target sometimes. With fast tear speed it's not that valuable as the tears don't even have time to find their intended targets before they hit the dank basement walls. What I'm trying to say with this useless wall of text is take it but it might not be as useful as you think.

    SV: 7

    Very good item to to synergize with. Not only does it make some god tier items mega-god tier, it also makes some pretty weak items (specifically, lazer items that aren't too good) good-god tier. It receives a 7 here because off the top of my head its synergies do require specific items that are small relative to the pool.


    4 CRICKET'S HEAD

    ItemID: 4

    "DMG up"

    • +0.5 Damage Up

    • +50% / 1.5 times Damage Multiplier (Does not stack with the Magic Mushroom multiplier).

    • Tears now have a significant knockback effect (Does not increase the shot speed stat).

    • Can be found in golden chests.

    • Cricket's Head used to be known as 'Max's Head in the original game and was renamed for Rebirth.

    TYPE: PASSIVE

    ITEM POOL: ITEM ROOM, GOLD CHEST

    GV: 8

    That's a lot of fucking damage. Even more if you already have a lot of fucking damage.

    SV: N/A


    5 MY REFLECTION

    ItemID: 5

    "Boomerang tears"

    • Gives tears a boomerang effect.

    • +1.5 Range Up.

    • +0.6 Shot Speed Up.

    • +1 Tear Height.

    TYPE: PASSIVE, TEAR MODIFIER

    ITEM POOL: ITEM ROOM

    GV: 1

    Its pretty bad. Not run ending per se but pretty bad. Generally I avoid this item. Your tears become shorter than a male genital in a freezing pool.

    SV: 0

    Generally fucks things up. Can be run-ending with the right (or wrong depending on how you look at it) items.


    6 NUMBER ONE

    ItemID: 6

    "Tears up"

    • Gives you a very high tear rate in exchange for a big range down.

    • +1.5 Tears Up.

    • -15.78 Range Down.

    • +0.45 Tear Height.

    TYPE: PASSIVE

    ITEM POOL: ITEM ROOM

    GV: 4-7

    Generally, I value damage output over range. It can be pretty bad if you already have bad range though. Tear height is... not that important. I would take this item if I have around normal tear rate, but at a tear rate of 7 I might think about not taking it.

    SV: 1

    Short range can be bad for synergies, but it's usually not run ending.


    7 BLOOD OF THE MARTYR

    ItemID: 7

    "DMG up"

    • +1.0 Damage Up.

    • +50% / 1.5 times Damage Multiplier if you also have Book of Belial (Does not stack with Magic Mushroom or Cricket's Head multipliers)

    • This item can only be found in the item room pool, unlike the original game where it was also a god item

    TYPE: PASSIVE

    ITEM POOL: ITEM ROOM

    GV: 7

    More damage is better damage.

    SV: 2

    It has an extremely rare synergy with Book of Belial but pretty insignificant in the grand scheme of things.


    BROTHER BOBBY

    ItemID: 8

    "Friends 'till the end"

    • A familiar which follows Isaac and shoots normal tears which do 3.5 damage.

    • Brother Bobby fires tears at a rate of 1 tear per second.

    TYPE: PASSIVE, FAMILIAR

    ITEM POOL: ITEM ROOM, DEVIL ROOM

    GV: 5

    Ok for early floors but useless later on. Can also make better familiars harder to control.

    SV: 2

    Better with BFF and whatever item it is that makes your babies home in on the enemy. Also good with Extension cord, but then again, all babies are. From now on general baby synergies with these items will not be noted.


    9 SKATOLE

    ItemID: 9

    "Fly love"

    • All fly enemies are no longer aggressive towards Isaac.

    • Is only available from the shell game in the arcade.

    • Can be won from the arcade skull game.

    • Picking up this item as well as 2 other fly-type items will allow you to transform into Lord of the Flies.

    TYPE: PASSIVE

    ITEM POOL: NONE (ARCADE SHELL GAME ONLY)

    GV: 6

    Its a good item to have with virtually no downsides.

    SV: 5

    Lord of the Flies is nice.


    10 HALO OF FLIES

    ItemID: 10

    "Projectile protection"

    • Gives Isaac 2 orbital flies which block enemy shots.

    • Picking up this item as well as 2 other fly-type items will allow you to transform into Lord of the Flies.

    TYPE: PASSIVE, ORBITAL

    ITEM POOL: ITEM ROOM, KEY BEGGAR

    GV: 6

    Pretty good shot-blocker for you. Just take it.

    SV: 5

    Lord of the flies is nice.


    11 1UP

    ItemID: 11

    "Extra life"

    • Gives the player an extra life

    • When revived, the green mushroom that follows Isaac will disappear and he will respawn with the same amount of red heart containers at full health.

    • Resurrection takes place before all other items which give you an extra live, except Guppy's Collar.

    TYPE: PASSIVE

    ITEM POOL: SECRET ROOM

    GV: 7

    Basically a Jesus hack.

    SV: N/A


    MAGIC MUSHROOM

    ItemID: 12

    "All stats up!"

    • +1 Health Up.

    • +0.3 Damage Up.

    • +50% / 1.5 times Damage Multiplier (Does not stack with the multipliers from Cricket's Head / Blood of The Martyr + Book of Belial).

    • +5.25 Range Up.

    • +0.3 Speed Up.

    • +0.5 Tear Height.

    • Increases the size of your player sprite, but doesn't increase the hitbox.

    • Similar to the original game, the Magic Mushroom does not give a tears up despite saying 'All Stats Up'.

    • Fully restores all red heart containers.

    • Can be dropped when exploding mushrooms in the Caves/Catacombs.

    TYPE: PASSIVE

    ITEM POOL: ITEM ROOM, BOSS ROOM, MUSHROOMS

    GV: 9

    Who doesn't want more stats? It fucking lies to you though, give me that fucking fire rate up Edmund you egg.

    13 THE VIRUS

    ItemID: 13 "Poison touch"

    • Deals poison damage on contact with enemies (Damage over time 4 or 6 per tick).

    • -0.1 Speed Down. (Cached: Only applied after the speed stat is next updated)

    • Has a chance to drop from the Lust miniboss fight.

    In Afterbirth this item can cause enemies to drop black hearts if it was poisoned.

    TYPE: PASSIVE

    ITEM POOL: ITEM ROOM, DEMON BEGGAR, LUST MINIBOSS

    GV: 4

    Situational. Falling below .9 speed can be bad, so all speed down items should be treated cautiously. I generally don't take this. Though the image of a small boy thrusting the corona virus into monsters is pretty funny.

    SV: 6

    Spun is nice. Doesn't really have bad synergies, can poison enemies with invincible items. Then again, poison generally isn't that strong with low damage. With high damage... you already have high enough damage to kill enemies anyway.


    14 ROID RAGE

    ItemID: 14

    "Speed and range up"

    • +0.6 Speed Up.

    • +5.25 Range Up.

    • +0.5 Shot Height.

    TYPE: PASSIVE

    ITEM POOL: ITEM ROOM, BOSS ROOM, DEMON BEGGAR

    GV: 7

    It's good. Getting it from a demon beggar when you already have high speed is like a slap in the face though, especially if you sacrificed almost all of your HP for it.

    Spun is nice.

    • +1 Health Up

    • Fully restores all red heart containers

    • Has a chance to drop from the Gluttony miniboss fight.

    Is bad if you want to maintain your red hearts at 1 or 0.5. Doesn't have good synergies. All healing items will not be explained further


    16 RAW LIVER

    Well I did a normal Lazarus run until I got steam sale and restock. Everything was normal until I got to Hush. I went into the shop and like normal I waited for items. I got missing no and it removed my steam sales and restock. I went to sheol and because of missing no this happened...btw I beat MS
